Orofacial Harmonization (HOF) is a segment that works as a complement to Dentistry and aims to promote balance and harmony in the smile and face by manipulating muscle, fat and bone structures, encompassing therapeutic, functional or aesthetic procedures. For this specialty, the face is considered as a whole, going beyond the oral cavity.
Orofacial harmonization can consist of the use of botulinum toxin (BT) - a purified substance that blocks muscle signals - or filling with hyaluronic acid (HA), a component that helps maintain tissue hydration.
Botulinum toxin
Botulinum toxin aims to attenuate or soften wrinkles and expression lines. This treatment consists of introducing small doses of this substance into the skin, allowing the facial muscles to relax and helping to harmonize the face.
This procedure is usually carried out on the upper third of the face, namely the forehead, glabella (the area between the eyebrows), and the corners of the eyes ("crow's feet"). It is also often used for smiles that expose the gums too much.
Filling with hyaluronic acid
Hyaluronic acid is an active compound present in our body that promotes skin elasticity and hydration. However, its natural production decreases with age.
Filling with hyaluronic acid is generally used in the middle and lower third of the face, namely the lips, malar (cheekbones), jawline, chin and dark circles. With the application of hyaluronic acid, it is possible to reduce the depth of wrinkles and correct some asymmetries.
Can restore volume and definition to the lips, giving the face a more youthful and attractive appearance.
Botulinum toxin and hyaluronic acid fillers can correct inverted smiles (when the smile line is curved downwards) and gummy smiles (when the gums are excessively exposed when smiling).
Improved skin elasticity, firmness and laxity.
It can smooth out wrinkles, fine lines, acne scars and dark spots, restoring youthfulness and vitality to the skin.
Can help relieve the symptoms of bruxism through treatment with botulinum toxin, used for muscle relaxation effect.
When there is excessive exposure of the gums when smiling, known as a gummy smile, this can affect facial aesthetics. Applying botulinum toxin to certain muscles can help reduce this exposure and correct the gummy smile.
Hyaluronic acid fillers can be applied to specific areas of the face to restore lost volume and smooth out irregularities. This can help correct asymmetries in the lips, cheeks and other areas around the mouth, providing a more balanced smile.
Bruxism, characterized by teeth grinding or clenching, can lead to tooth wear, jaw pain and headaches. Applying botulinum toxin to certain jaw muscles can help reduce muscle activity and relieve the symptoms of bruxism.

The Orofacial Harmonization procedure is a non-invasive process that makes it possible to delay and attenuate the signs of the natural aging process. However, there are some contraindications for certain types of patients, such as:
- Women who are pregnant or breastfeeding;
- Immunosuppressed patients;
- People suffering from autoimmune diseases.
Generally speaking, there are no age limits for starting Orofacial Harmonization treatment, and it can be carried out by anyone looking for a rejuvenated and more harmonious appearance of their facial morphology. However, an initial medical assessment of each case is always necessary in order to determine whether the treatment is suitable for the patient in question.
The duration of the Orofacial Harmonization treatment varies according to the type of procedure and is always defined at the first assessment appointment. Even so, treatment times vary between 30 and 40 minutes, and some are performed under local anesthesia.
Also depending on the type of procedure performed, the results of Orofacial Harmonization tend to appear quickly - after 3 to 5 days - in the case of TB, and in some cases touch-ups may be necessary. In the case of HA, the effect of the filler is immediate, but sometimes, due to edema, the final result is obtained after a few weeks.
In order to promote facial harmony and, consequently, better self-esteem for patients, it is essential to maintain good oral hygiene habits in conjunction with the treatments, so as to achieve a healthy smile that promotes general health.
